More hospitals and healthcare providers are exploring, testing or implementing Hospital@Home care pathways in which (hybrid) hospital care is provided at home. An important step towards future-proof and patient-centered care delivery.
This questionnaire aims to better understand how H@H is being defined, organized and delivered in various countries worldwide. The questionnaire is intended for anyone involved in H@H programs – such as innovation managers, project leaders, healthcare professionals, and others in similar roles.
The results will capture the similarities, differences, strengths, and challenges of H@H programs worldwide. Participants can leave their email address at the end of the questionnaire to receive the results.
